Overview
The Family Medicine Physician provides comprehensive, patientcentered primary care services to individuals and families across the lifespan in a Federally Qualified Health Center (FQHC) setting, including service delivery via a mobile health unit. This role focuses on improving access to care for underserved and highneed populations through preventive care, chronic disease management, and acute care services. The physician works collaboratively with an interdisciplinary care team and supports the FQHC mission of delivering highquality, culturally responsive care.

Qualifications
Education & Licensure
- MD or DO from an accredited medical school
- Completion of an accredited Family Medicine residency program
- Boardeligible or boardcertified in Family Medicine
- Active or eligible for active Georgia medical licensure
- DEA license or ability to obtain
Experience & Skills
- Experience working in primary care, community health, or FQHC settings preferred
- Comfort providing care in mobile, communitybased, or nontraditional clinical environments
- Strong commitment to serving underserved and diverse patient populations
- Excellent interpersonal, communication, and teamwork skills
- Ability to adapt to varying locations, schedules, and patient needs
- Bilingual skills preferred but not required
Responsibilities
- Provide comprehensive primary care services including preventive care, acute illness management, and chronic disease treatment for patients of all ages
- Deliver care in a mobile health unit setting and, as needed, at fixed clinic locations
- Perform patient assessments, diagnoses, treatment planning, and followup care
- Provide patient education focused on health promotion, disease prevention, and selfmanagement
- Accurately document care in the electronic health record in accordance with FQHC, regulatory, and payer requirements
- Collaborate with nurses, medical assistants, patient registration staff, care coordinators, and community health workers
- Support continuity of care through appropriate referrals and coordination with specialty and community resources
- Participate in team meetings, case conferences, and care planning activities
- May serve as a clinical resource or mentor to advanced practice providers and clinical staff, depending on program needs
