Credentialing: 60 days
Board Certified/Board Eligible: BC/BE in ABEM (or osteopathic equivalents)
Certifications: ACLS, ATLS ( If ABEM, need current ACLS and 1X proof of ATLS )
Licensure: Active TX State Medical License, DEA with TX address
