Salary : $253,049/Annually - Salary is Negotiable (Employees will receive 5% below the negotiated pay rate during their probationary status.)

Responsible To : Medical Director

Summary : Provides and manages direct medical evaluation, diagnosis, procedures, and care for patients within a recognized area of subspecialty practice. Provide consultation and advice to others as a highly specialized practitioner. Continuation of this position is contingent upon funding allocations.

• Medical Doctors degree: Wisconsins Physician License.
• State of Wisconsin and Federal DEA Certification.
• Medical Specialty License or Certification.
• Board certified or Board Eligible in Specified Area of Medical Specialty.
• Must be credentialed.

• Employees are required to have proof of seasonal influenza vaccination; medical or religious exemption may be requested upon hire.
