Salary : $210,874 /Annually - Salary is Negotiable (Employees will receive 5% below the negotiated pay rate during their probationary status.)

Responsible To : Medical Director

Summary : Under direct supervision of the Medical Director, provides independent primary care in a variety of clinical settings to a specified population of non-emergency patients. Provide consultation and advice to others as a highly specialized practitioner. Continuation of this position is contingent upon funding allocations.

• Medical Doctor degree; Wisconsins Physician License
• State of Wisconsin and Federal DEA Certification.
• Medical Specialty License or Certification.
• Board certified or Board Eligible in Specified Area of Medical Specialty.
• Must be credentialed.
