Role Description
The Primary Care Physician will be responsible for diagnosing and treating acute conditions, conducting Annual Wellness Visits (AWVs), and coordinating specialist referrals. This role emphasizes preventative care, personalized service, and building strong, long-term patient relationships.
Key Responsibilities
- Diagnose and manage acute and chronic medical conditions
- Perform Annual Wellness Visits (AWVs) and routine check-ups
- Develop and implement preventative care plans
- Coordinate and manage specialist referrals
- Maintain accurate and compliant documentation, including quality metrics (eg, HEDIS)
- Provide patient-centered care with a focus on long-term health outcomes
- Build and maintain strong patient relationships within a concierge care model
Required Skills
- Strong clinical assessment and diagnostic skills
- Preventative care and wellness management expertise
- Proficiency in conducting Annual Wellness Visits (AWVs)
- Knowledge of HEDIS measures and quality reporting standards
- Care coordination and referral management skills
- Excellent patient communication and relationship-building abilities
- Strong documentation and EMR/EHR proficiency
- Ability to manage geriatric/senior patient populations
- Critical thinking and clinical decision-making skills
- Time management and ability to work in a personalized, concierge care setting
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school
- Active and unrestricted medical license in the state of practice (California preferred, if applicable)
- Experience in primary care, preventative medicine, or wellness-focused practice
Preferred Qualifications
- Background in Family Medicine or Internal Medicine
- Experience working with managed care populations, especially seniors
- Knowledge of quality documentation and HCC coding
