Requirements:

Active VA License or IMLC

BC

ACLS

DEA

Clean Record

NPDB Report

Local candidate (highly preferred)

AHF Fellowship - LVAD experience

Must be Ventricular Assist Device (VAD) certified (for Advanced Heart Failure)
